I made this formula for myself first

At thirty, a slipped disc and sciatica put me in a situation I had not seen coming. For several weeks, sitting became the position I avoided: meals standing up, shorter journeys, calls taken leaning against a wall. It is not a dramatic story. It is an ordinary day whose options shrink one by one.

That is where this formula comes from, not from a range plan. I started reading labels, a lot of them, looking for one specific thing: what form an active was used in, and at what amount for a whole day. I almost never found it. A plant name in large letters, a figure with nothing to compare it to, and for the rest you had to take it on trust.

What stopped me was not the quality of these products, which I had no way of judging. It was exactly that: no way of judging. I turned over dozens of boxes looking for something to compare them by, and each time I came away feeling I had read a sales pitch rather than a composition.

So we wrote the page I had not found. Five actives, their extraction form, their standardisation and their daily amount, published on this page and not only on the back of the bottle. Four capsules a day, because these amounts do not fit into one. And nothing beyond that: no health claim is authorised on these plants in Europe, for any brand. What is left for me to do is tell you what is inside.

SciaticaWhat the regulations forbid us to write, and what they let us show

Sciatica is not a more intense form of lower back pain. It is damage to a nerve root. Assurance Maladie, France’s national health insurance body, describes its most frequent cause: the nucleus of the intervertebral disc “can migrate within the outer ring and then bulge into the spinal canal, forming a lumbar disc herniation,” and “the pain is caused by compression, by this herniation, of one of the roots of the sciatic nerve, combined with its inflammation.”

Its management is described by the health authorities, and it includes no food supplement: medicines, staying active, rehabilitation once the acute phase has passed, and surgery in certain forms. France’s National Authority for Health (HAS) also notes that “physical activity is the main treatment enabling a favourable course of lower back pain and preventing a recurrence.”

On plants, European regulation has never settled the matter. The European Food Safety Authority states that 1,548 claims relating to botanical substances were “put on hold,” pending a final decision from the Commission and the Member States. No health claim is therefore authorised to date on curcumin or boswellia, not for us or for anyone. All that is left to us is the composition. That is what this page publishes.

What is sciatica, and how does it differ from lower back pain?

Assurance Maladie, France’s national health insurance body, defines it as follows: “Sciatica is the name given to neuralgia of the sciatic nerve. It refers to pain in a lower limb, along the path of this nerve, often associated with lower back pain. This is then called lumbar sciatica.”

The distinction lies in the path. “Back pain, or lower back pain, stays localised to the lower part of the back or lumbar region,” the same source notes, whereas sciatica travels down the lower limb. The nerve involved “provides both sensation and certain movements to part of the lower limb: it is both sensory and motor. It is the largest and longest nerve in the body.”

Why is the pain not in the same place for everyone?

Because the nerve has two main roots and the path depends on which one is affected. Assurance Maladie: “If the L5 root is affected, sciatic pain is located behind the thigh, on the outer side of the knee, on the outer side of the leg, the top of the foot and the big toe. If the S1 root is affected, sciatic pain is located behind the thigh, behind the knee, in the calf, the heel, the sole and the outer edge of the foot down to the last three toes.”

This mapping is what the doctor uses: after their examination, they “can tell you whether it is L5 or S1 sciatica depending on which nerve root is affected.”

What is the most common cause?

A slipped (herniated) disc. Assurance Maladie describes the mechanism: “When pressure on the disc increases, its central part, the nucleus, can migrate within the outer ring and then bulge into the spinal canal, forming a lumbar disc herniation, whose size varies considerably.” The pain then comes from “compression, by this herniation, of one of the roots of the sciatic nerve, combined with its inflammation.”

Inserm, the French national medical research institute, describes the same mechanics from the disc’s side: “Following repeated strain, trauma, or awkward movements, this ring cracks and lets the contents of the pulpy nucleus escape.”

One point is worth knowing, as it saves a lot of worry when reading an imaging report: “the presence of a disc herniation is not always associated with lumbar sciatica.”

Why does sitting come up so often on this topic?

Because it is explicitly among the situations where the pain varies. Assurance Maladie lists them: pain “is often triggered by exertion (when a person lifts a weight, for example)”; “it increases when sitting, during coughing, sneezing, or exertion”; “relief is most often felt lying down.”

For many readers, it is also their working position. The same source notes “that it is also essential to change position regularly, to get up, walk and stretch,” and details how to sit at a screen: “feet resting flat on the floor or on a footrest; the elbow angle is a right angle or slightly greater; forearms close to the body; the hand in line with the forearm; the back straight, supported by the chair back.”

Sciatica: in which cases should you seek emergency care?

This is the most important question on this page. Assurance Maladie advises seeing your GP urgently in the following cases:

  • “you feel extreme sciatic pain that is not eased by painkillers”;
  • “you notice paralysis or a reduction in muscle strength in your leg”;
  • “you notice a loss of sensation in your perineum (the area between the anus and the external genitals)”;
  • “you have difficulty urinating or urinary leakage”;
  • “you have lost control of the anal sphincter (involuntary passing of stools) or, conversely, unusual constipation”;
  • “you currently have, or have recently had, one of the following symptoms: fever, difficulty breathing, vomiting, urinary infection.”

If you recognise yourself in any of these, close this page and call a doctor. No food supplement has a place in this situation.

What is cauda equina syndrome?

It is one of the serious forms, and it is a medical emergency. Assurance Maladie describes three, “more serious, but much rarer”: paralysing sciatica, where “the pain is accompanied by motor deficit in the leg, with loss of the ability to make certain leg movements”; hyperalgic sciatica, where “the pain is intense and is not eased by usual painkillers”; and cauda equina syndrome.

In this last case, the person has “sensory disturbances in the leg and perineum,” “disturbances in sphincter control,” and “motor disturbances in the lower limbs with fatigue when walking, a decrease in muscle strength.” France’s National Authority for Health classes these signs among its “red flags”: “Extensive neurological symptom (deficit in bladder or anal sphincter control, motor impairment in the legs, cauda equina syndrome).”

Assurance Maladie notes that “surgery is urgent when there is paralysis, cauda equina syndrome, or when the sciatica is hyperalgic (extremely painful).” These are medical emergencies, not situations in which to look for a product.

What signs call for medical advice, without being an emergency?

Assurance Maladie lists five: “after a few days of self-treatment, your condition does not improve, the lower back pain persists and it significantly limits your daily activities”; “progressive weakness or numbness appears along the sciatic nerve in a leg or foot”; “you notice a decrease in sensation in part of the leg”; “the sciatic pain intensifies, especially at night”; “you have lost weight unintentionally, before the sciatica appeared.”

France’s National Authority for Health includes among its red flags “non-mechanical pain: pain that gradually worsens, present at rest and particularly at night,” “unexplained weight loss,” “history of cancer,” and fever.

How long does an episode of sciatica generally last?

France’s National Authority for Health gives a benchmark for common low back pain, which “may be associated with radicular pain, meaning pain in one or both lower limbs along one or more dermatomes”: “in 90% of cases, common low back pain resolves favourably within 4 to 6 weeks.” It notes that this outlook should be communicated to the patient: “It is essential to give reassuring information about the prognosis.”

This figure applies to common low back pain, defined as “lower back pain with no warning signs,” not to the more complicated forms mentioned above.

Why do episodes come back?

Recurrence is built into the terminology itself. France’s National Authority for Health proposes using the term “recurrent low back pain” “in the event of a recurrence of low back pain within 12 months,” and adds: “It should be considered low back pain at risk of becoming chronic.”

In other words, what is at stake is not just the current episode, but the gap between episodes. HAS places physical activity at the centre of that gap: “Physical activity is the main treatment enabling a favourable course of low back pain and preventing a recurrence.”

On the working side, Assurance Maladie mentions the pre-return-to-work visit with the occupational health doctor, which allows “an assessment of your fitness for your position and a proposal for ergonomic adjustments to your workstation to prevent recurrence, or a recommendation to change role.”

Should you stay lying down during an episode of sciatica?

No. Assurance Maladie is explicit: “prolonged bed rest is not advised.” It recommends, “if the pain is bearable,” to “continue and adapt your activities, without forcing it,” and notes elsewhere that “when suffering from sciatic nerve neuralgia, prolonged bed rest should be avoided. Muscle strengthening is essential.”

It adds that “it is possible to resume certain sporting activities, such as walking or swimming, without waiting for the lower back pain to disappear completely,” after speaking to your GP.

Can sciatica be recognised as an occupational illness?

Yes, under certain conditions. Assurance Maladie states: “Under certain conditions, sciatica can be recognised as an occupational illness if it is caused by manual handling of heavy loads. Speak to your doctor about it.”

This is information often unknown to those it concerns most directly, and it costs nothing to check with your GP or occupational health doctor.

Is sick leave automatic, and for how long?

It is not automatic, and its length depends largely on the job. Assurance Maladie states that “if necessary, your GP will prescribe sick leave,” of a duration that is “highly variable” and adapted to “your age and physical condition; the cause of your sciatica; the nature of your work; the time and means of getting to work.”

Two rough figures are given: “For sedentary work, 1 to 2 days of sick leave are usually sufficient. For very physical work involving carrying heavy loads, around five weeks of sick leave may be necessary.”

Sciatica: is an MRI or CT scan needed?

Not straight away. Assurance Maladie: “Initially, no further examination is useful. However, if the sciatic pain persists beyond a few weeks, if it resists treatment, or if you present a complicated form from the outset, further tests will be prescribed: X-rays of the dorsolumbar spine, CT scan, MRI…”

France’s National Authority for Health sets the same rule and explains why: “In the absence of a red flag, there is no indication for spinal imaging in the case of an acute flare-up of low back pain with or without radicular pain, given the lack of systematic correlation between imaging and clinical findings.” What the image shows does not always match what is felt.

Can a clinical examination reliably identify the cause?

No, and a Cochrane review has measured this. It pooled nineteen studies on physical examination tests used to identify lumbar radiculopathy due to disc herniation. Its conclusion: “When used in isolation, current evidence indicates poor diagnostic performance of most physical tests used to identify lumbar disc herniation.”

The authors add two nuances: most results come from populations already referred for surgery, and “better performance may be obtained when tests are combined.” The same review also notes that “most patients will respond to conservative treatment.”

Can a food supplement replace a medical treatment or medical advice?

No. None of the care pathways described by the health authorities includes a food supplement: not the Assurance Maladie pages on sciatica, nor the memo from France’s National Authority for Health. What appears there are painkillers, non-steroidal anti-inflammatory drugs, muscle relaxants, injections, staying active, rehabilitation once the acute phase has passed, and, in certain forms, surgery.

ANSES, France’s food safety agency, also reminds consumers of the importance of “discussing with a health professional whether it is appropriate to consume a food supplement given their state of health,” of “reporting the consumption of food supplements and any concurrent medical treatment to their doctor or pharmacist, owing to the risk of interaction,” and of “avoiding consuming several food supplements at the same time.”

Does turmeric carry any risks?

Yes, and stating this on the page of a product that contains it seems the least we can do.

ANSES, France’s food safety agency, reports that the French and Italian vigilance schemes “have recorded more than 40 cases of hepatitis occurring between 2002 and 2021 during the consumption of food supplements containing turmeric or curcumin, for which causality is considered likely or very likely in 9 cases.” The Agency “advises against the consumption of turmeric by people suffering from disorders of the biliary tract” and considers that this consumption “should be avoided in people treated with anticancer medicines or medicines with a narrow therapeutic margin, such as anticoagulants.”

On amounts, the expert assessment “determined that taking such supplements raises no particular concern as long as consumption does not exceed an intake of 153 mg of curcumin per day for a 60 kg adult.” With a caveat that concerns us directly: “the ADI set by EFSA does not apply to formulations in which the bioavailability of curcumin has been increased.” This formula provides 92 mg of curcuminoids per daily dose and uses an encapsulated form. Following the stated dose and seeking medical advice if you are on ongoing treatment is therefore not a stylistic flourish.

On the topic of joints, European regulation authorises one statement under the pending claims: boswellia contributes to the health of joints and bones. This formula provides 307 mg per daily dose, as a concentrated extract standardised to 65% boswellic acids, providing 200 mg of boswellic acids.

Beyond this sentence, nothing more can be stated, by us or by anyone else.

And turmeric, what can be said about it?

The same framework applies: turmeric contributes to joint health. This formula provides 400 mg of Longvida® curcumin, including 92 mg of curcuminoids, extracted using the patented “Solid Lipid Curcumin Particle” technology.

As ordinary curcumin is very poorly absorbed, it was the form that guided this choice, not the raw quantity. Here again, nothing else can be stated.
